Do I need a doctors referral to start physical therapy?

No, Illinois has Direct Access! Which means you do not need a referral from a doctor to receive physical therapy care. However, it is always recommended to have contact with your physician or dentist during care.

Do you take insurance?

Achieve Physical Therapy is an out-of-network provider. You can utilize cash, check, zelle, venmo, card, and HSA/FSA to cover services. We can provide you a SuperBill upon request to submit to insurance for out of network reimbursement.

Payment is due at the time of service: we accept all major credit cards, a 3% processing fee will apply.

If you are not completely satisfied with our services you will be refunded 100% of payment.

Planning for your first appointment?

If you are thinking about coming in for physical therapy - know that we are here to listen and move forward together. At Achieve we have the 1 on 1 space to take our time with appointments, you will have time to ask questions and for us to build a program that makes sense for you.

Prior to your first visit, you will receive a patient intake form to complete. When coming in for your first visit, please wear athletic clothing in order for our therapists to get a good idea of how you are moving.

What should I expect at my first visit?

At the initial evaluation we will take range of motion measurements, muscle testing, and specific joint testing to assess your injury. You will come away from your first visit with education on the components of your injury, a plan moving forward, and home exercises to complete in between your appointments.

Are these home exercises really going to help me?

YES! Performing your home exercise program reinforces what we have worked on during your session, helps prevent reinjury, and shows in research studies to reduce pain and improve long term outcomes. So, you can GRADUATE from physical therapy!

Cancellation Policy

At Achieve Physical Therapy we are committed to providing the highest quality of care and ensuring that appointment times are reserved exclusively for you. To help us maintain this standard, we ask that you please provide at least 24hr notice if you need to cancel or reschedule your appointment. We understand unforeseen circumstances may arise, therefore your first missed appointment will not incur a fee. This is our courtesy exception. Subsequent appointments will be charged the full cost of the scheduled session.  You may cancel or reschedule by calling our office at 8473873610 or emailing our staff at Julie@achieveptwellness.com

HIPAA Policy

Achieve Physical Therapy is committed to protecting the privacy and security of your health information. This notice explains how we safeguard your Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA).

We may collect PHI when you:

  • Schedule an appointment

  • Complete intake or medical history forms

  • Communicate with us by phone, email, or through our website

  • Receive treatment or services from our practice

PHI includes information such as your name, date of birth, medical history, treatment information, insurance details, and any other data used to identify you.

We may use or disclose your PHI for the following purposes:

  • Treatment: Coordinating or managing your care with physicians or other healthcare providers.

  • Payment: Processing payments for services.

  • Healthcare Operations: Quality improvement, staff training, compliance activities, and administrative functions.

We will not use or disclose your PHI for any other purpose without your written authorization unless required or permitted by law.

You have the right to:

  • Access and request copies of your health records

  • Request corrections to your information

  • Request restrictions on how your information is used or shared

  • Request confidential communication methods

  • Receive an accounting of certain disclosures

  • File a complaint if you believe your privacy rights have been violated

We will not retaliate against you for filing a complaint.

We use administrative, technical, and physical safeguards to protect your PHI, including:

  • Secure electronic systems

  • Limited access to patient information

  • Staff training on privacy and security practices

  • Policies to prevent unauthorized use or disclosure
