Achieve Physical Therapy & Wellness ("Achieve Physical Therapy," "we," "us") respects your privacy. This policy explains what information we collect through achieveptwellness.com, how we use it, and the choices you have.

This policy covers our website only. Health information we create or receive while providing care to you as a patient is protected under HIPAA and is described in our Notice of Privacy Practices, which is provided at your first visit and is available on request.

Information you give us

  • Contact form. When you send us a message we collect your first and last name, email address, the type of visit you are interested in, your area of concern, and whatever you choose to write about your injury or goals. Please share only what you are comfortable sending through a website form.
  • Jaw Pain Starter Guide and email sign-ups. When you request our free guide, or check the box to receive news and updates, we collect your email address so we can send you the guide and occasional emails from the practice.
  • Phone and email. If you call or email us directly, we receive whatever you choose to share.
  • Online booking. Our "Book" buttons take you to IntakeQ, a separate scheduling and practice management service. Information you enter there is collected by IntakeQ on our behalf and is handled under its own privacy policy and our Notice of Privacy Practices.

Information collected automatically

Our website uses cookies and similar technologies to help us understand how the site is used and how people find us. Analytics cookies are placed only if you accept them. Advertising measurement is on by default, and you can turn it off at any time as described below. Depending on your cookie choices, this may include:

  • Pages viewed, links and buttons clicked, time on the site, and the site or ad that referred you
  • Device, browser, and approximate location derived from your IP address
  • Session recordings and heatmaps that show how visitors scroll and click, with text entered into form fields masked

  • Google Ads (measuring whether our ads lead to visits and inquiries)
  • Microsoft Clarity (session recordings and heatmaps)
  • Ahrefs Web Analytics (aggregate traffic measurement)
  • Google Maps (the embedded map on our Contact page)

Each of these providers processes data under its own privacy policy.

How we use information

  • To send the guide or emails you asked for

We do not sell your personal information. We share it only with the service providers listed above, who process it on our behalf, or when the law requires us to.

Your choices

  • Cookies. On your first visit, a banner asks about cookies. Analytics cookies are placed only if you select Accept all. Advertising measurement is on until you decline. You can decline or change your choice at any time with Decline all or Manage cookies in the banner, or the Cookie Preferences button at the bottom of the screen. When cookies are declined, our analytics and advertising tools do not set cookies on your device, though Google may still receive limited signals without cookies, such as that a page was viewed. You can also clear or block cookies in your browser settings.
  • Email. Every marketing email includes an unsubscribe link. You can also ask us to remove you at any time.
  • Access and deletion. You can ask us what website information we hold about you, or ask us to correct or delete it, by contacting us below.

How long we keep information

We keep contact form messages and email sign-ups for as long as needed to respond to you and to operate the practice, or until you ask us to delete them. Analytics data is kept according to the retention settings of each provider.

Children

This website is intended for adults. We do not knowingly collect personal information online from children under 13. A parent or guardian should contact us on a child's behalf.

Changes to this policy

We may update this policy from time to time. The effective date at the top of the page shows when it was last revised.
